What to Bring to the Appointment Policy, Payment and Fees Emergencies Hospital Affiliations After Hours Calls Prescription Renewals Dr. Chappidi and Dr. Tilwalli usually see patients by appointment. Appointments can be made by calling their office at 773.205.9800. The doctors request that if patients are unable to keep their appointments, they kindly remember to cancel so that others may use the allotted time. What to Bring to the Appointment
We accept a variety of insurance plans. However the patient is asked to pay his/her insurance plan's deductible and co-payment at the time services are rendered. Personal checks are accepted. Dr. Chappidi and Dr. Tilwalli are on staff at Our Lady of the Resurrection Medical Center and Saints Mary and Elizabeth Medical Center. In case of an emergency, the patient (or a designee) should call the office and speak with the physician-on-call to arrange for service in the office or in the Emergency Room at Our Lady of the Resurrection Medical Center or Saints Mary and Elizabeth Medical Center. If there is a life-threatening situation, the patient should call "911" or go directly to the Emergency Room and have a designee call the office from there. Hospital Affiliations Dr. Chappidi's and Dr. Tilwalli's concern for their patients does not stop when they leave the office. If there is an emergency or serious medical problem requiring immediate care after hours, the patient or designee should call the office number. The answering service will take the caller's name and telephone number and contact the doctor or the physician-on-call who will respond as soon as possible. Prescription renewals are authorized during office hours. The patient is asked to have the pharmacist's name and telephone number handy when calling the office for a renewal. |
