Appointments

Dr. Chappidi usually see's patients by appointment. Appointments can be made by calling the office at 773.205.9800. Dr. Chappidi requests that if patients are unable to keep their appointments, they kindly remember to cancel so that others may use the allotted time.


What to Bring to the Appointment

  • Patient's insurance card and photo ID
  • Physician referral form, if required by your insurance provider
  • A list of medications, including the dose and frequency
  • Pertinent information about the patient's medical and surgical history
  • Any recent EEG's, EMG's, CAT scans, or other test results related to your condition
  • Completed patient registration form

Policy, Payment and Fees

We accept a variety of insurance plans. However the patient is asked to pay his/her insurance plan's deductible and co-payment at the time services are rendered. Personal checks are accepted.


Emergencies

Dr. Chappidi is on staff at Community First Medical Center, Saints Mary and Elizabeth Medical Center, Resurrection Medical Center, Saint Francis Hospital, and Illinois Masonic Medical Center. In case of an emergency, the patient (or a designee) should call the office and speak with the physician-on-call to arrange for service in the office or go to the closest emergency room and have the neurologist paged/informed if needed. If there is a life-threatening situation, the patient should call "911" or go directly to the Emergency Room and have a designee call the office from there.

After Hours Calls

Dr. Chappidi's concern for his patients does not stop when he leaves the office. If there is an emergency or serious medical problem requiring immediate care after hours, the patient or designee should call the office number. The answering service will take the caller's name and telephone number and contact the doctor or the physician-on-call who will respond as soon as possible.


Prescription Renewals

Prescription renewals are authorized during office hours. The patient is asked to have the pharmacist's name and telephone number handy when calling the office for a renewal.
