We accept a variety of insurance plans. However the patient is asked to pay his/her insurance plan's deductible and co-payment at the time services are rendered. Personal checks are accepted. Dr. Chappidi is on staff at Community First Medical Center, Saints Mary and Elizabeth Medical Center, Resurrection Medical Center, Saint Francis Hospital, and Illinois Masonic Medical Center. In case of an emergency, the patient (or a designee) should call the office and speak with the physician-on-call to arrange for service in the office or go to the closest emergency room and have the neurologist paged/informed if needed. If there is a life-threatening situation, the patient should call "911" or go directly to the Emergency Room and have a designee call the office from there. Hospital Affiliations
Dr. Chappidi's concern for his patients does not stop when he leaves the office. If there is an emergency or serious medical problem requiring immediate care after hours, the patient or designee should call the office number. The answering service will take the caller's name and telephone number and contact the doctor or the physician-on-call who will respond as soon as possible. Prescription renewals are authorized during office hours. The patient is asked to have the pharmacist's name and telephone number handy when calling the office for a renewal. |
